Boston University professor crushed to death in an elevator in her apartment building last month was hit by the large package she was carrying at the time, state inspectors found

The 2A 29m-high box hit a switch that caused the elevator to fall while the door was still open – and Carrie O’Connor, 38, fell into the shaft, ultimately resulting in her death on September 14, according to a report from the Massachusetts Office of Public Safety and Inspections

While inside the elevator, a maintenance person in the basement pressed the call button – signaling the system to lower the elevator as soon as the interior door was closed, according to report

Pictures showed the package then banging on the car door switch – mistakenly indicating the door was closed – and the elevator began to fall with the interior door open

When Mrs O’Connor lost control of the box, she slipped the switch, interrupting the descent of the elevator But when she then lifted the package again, according to the video, it knocked once moreover the switch

She then disappeared from view in the footage, indicating « she had fallen backwards into the lift path between the 1st floor and the basement, » the report says

« I heard someone bringing a package down the hall, then I heard an unholy scream, » said Leanne Scorzoni, a resident of ABC-TV at the time.

« Then we rushed down the hall and saw a man obviously in distress He was screaming and hyperventilating, saying, ‘She’s dead! She is dead! »»
